Supporting Adults with a range of Diverse and Complex Needs

We support adults (18–64) and older adults (65+) with a wide range of needs, including mental health conditions, dementia, physical disabilities, sensory impairments, and individuals who require personal care or daily living support. Our care is compassionate, consistent and focused on maintaining dignity, independence and wellbeing — at home or in supported living.

We offer this support in both shared supported living homes and in people’s own homes across the community.

Our support is tailored to individuals who:

• May need daily personal care and medication support
• May have mental health needs or be living with dementia
• May have physical or sensory disabilities affecting mobility, safety or routines
• May be returning home from hospital or moving into supported living
• Want to build confidence, keep community links and live as independently as possible

We accept referrals from local authorities, NHS Trusts, Integrated Care Boards (ICBs), and adult social care teams. Every individual we support is assessed holistically to ensure we can safely, compassionately, and effectively meet their needs.
